Precautionary Coronavirus Liability Release Form

Due to the 2019-2020 outbreak of the novel Coronavirus, COVID-19, we are taking extra precautions with the intake of each volunteer, guest, staff member, health history review, as well as sanitation and disinfecting practices. 

Symptoms of COVID-19 may include: Fever, Fatigue, Dry cough, Difficulty breathing

I agree to the following:

  • I understand the above symptoms and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above within the last 14 days.
  • I affirm that I, as well as all household members, have not been diagnosed with COVID-19 within the last 30 days.
  • I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID 19 within the last 30 days.
  • I affirm that I, as well as all household members, have not traveled outside of the country, or to any city that is or has been considered a "hot spot" for COVID-19 infections within the last 30 days.
  • I understand that Oceanside Museum of Art, staff personnel, guests, and other volunteers cannot be held liable for any exposure to the virus or any other contagion caused by misinformation on this form or the health history provided by each individual.

By dating and submitting the application below I agree to each above statement and release Oceanside Museum of Art from any and all liability for the unintentional exposure or harm due to COVID-19.

Oceanside Museum of Art and all employees of this facility agree that they abide by these same standards and affirm the same. They also affirm that we have improved and expanded our sanitation protocols to more thoroughly fight the spread of COVID-19 and other communicable conditions and by dating and submitting below, you agree to follow the safety guidelines and protocols implemented to continue the safety of all.

Staff and volunteers work together to make up the human resources of the museum. As such, volunteers make the following pledge:

  • Assist the museum to the best of my ability
  • Honor the mission, vision, values of the volunteer program
  • Keep a current application on file with the volunteer office and inform the museum of any changes
  • Attend volunteer training sessions, events, and activities
  • Read and abide by OMA’s volunteer handbook
  • Abide by the museum’s policies and procedures
  • Track my volunteer hours and keep abreast of information online and at the front desk
  • Be on time and proactive in handling work
  • Keep the schedule I commit to, or find a replacement if I cannot come in
  • Keep up to date on new volunteer opportunities
  • Work collegially and effectively with my volunteer peers and staff
  • Give feedback to the staff when I feel it is warranted
  • Keep museum matters confidential
  • Be a cheerful and an informed advocate for the work of the museum
  • Present myself in a professional manner
